Electronically Fillable Forms Now Available!

You can now complete and send your PBR forms entirely electronically.

As part of a comprehensive review of its service delivery arrangements, IP Australia has re-developed the entire suite of PBR forms making the process entirely electronic. Customers can now easily complete the forms on-line and even submit the form to IP Australia electronically.

All forms are now available in this new format, making the whole process convenient and easy to use!

The forms themselves have not changed greatly, the main difference you will notice is signatures are no longer required. This has allowed the forms to be totally fillable on-line. Click here for instructions on how to complete the forms or contact IP Australia’s Customer Service Centre on 1300 65 10 10 should you require help completing the forms.

Making a Payment

Never send credit card information to IP Australia by email since the security of your credit card details or other payment information cannot be ensured outside IP Australia’s infrastructure.

Please Note:

  • To avoid processing delays, please do not use old forms. It is recommended that the most recent version of a form be submitted as per the forms listed below.
  • Form PBR00008 ‘Confirmation of Submission of ACRA Specimen’ has been removed. This form is no longer required.
  • Receipt of an email occurs on the date and time the entire email first enters IP Australia’s information system at the Central Office in Canberra, ACT. It is not the time or the date the email is sent. The date and time of receipt is based on Australian Eastern Standard/Daylight Time (AEST/AEDT).

Instructions for completing the new PBR Forms

All PBR forms can now be completed electronically and sent via email to IP Australia. This streamlined approach was made possible by removing the requirement for hand written signatures on the forms. The need for signatures has been replaced by a declaration style check box which, under the Electronic Transaction Act and PBR legislation, provides a means of identifying the person submitting the form.

Filling out the forms

The fillable PDF forms contain a number of field types. The most common being text boxes (for entering text or details), tick boxes (for selecting an option) and date fields (which display a calendar for you to select the appropriate date). Some forms also have drop down lists which you can select from, for example selecting your State or Territory.

To complete the declaration on any of the forms, simply tick the check box and enter your full name in the name field. Some declarations will also ask you to identify your role or interest in the application, for example Applicant/Agent or Qualified Person. See the example declaration below.

If you experience difficulties in completing the form, please contact IP Australia on the details below, and in the meantime print the form and fax or send directly to IP Australia.

Sending the forms

To send single forms via email simply click on the ‘submit via email’ button at the end of the form. This will automatically attach the completed form to an email and pre-populate the address to assist@ipaustralia.gov.au. If required you can then add any general text to the body of the email and click send.

For attaching multiple forms to an email, users are required to save the completed forms to their local drive and attach them using the existing attach function within their email client.
