Please complete this form when authorising a person/entity to act on behalf of an applicant in relation to a PBR application.
This form is used by the Qualified Person (QP) after completion of the Application Part 2 to certify the work they have undertaken.
Please complete this form for confirmation of submission of a satisfactory specimen to the Herbarium.
Before a grant of PBR can be made, it is a requirement that propagating material of the variety must have been deposited for storage, at the expense of the applicant, in a genetic resources centre (GRC)

Please complete this form when making an application for a declaration of essential derivation. The PBR Act allows grantees of PBR varieties to apply for a declaration of Essential Derivation (ED) where they believe that another variety meets the ED criteria as outlined in section 4 and 40.
Please also complete this form when applying for a PBR (ie Part 1 General Information form) or when changing the nominated qualified person for an existing plant breeder's rights application.
This Form is used where there is more than one person (or entity) is applying as applicants for PBR protection for a single variety. The Form collects formality data and the relationship of this person/entity with the breeder of the variety. If you are applying through PRISMA you do not need to complete this form as it is already built into the system.
